Founded in 2020, our organization was built on a simple but powerful conviction, that quality, compassionate care shouldn’t depend on your resources, and that every person, regardless of their situation, deserves to feel at home.

What started as one group home has grown into one of Wisconsin’s fastest-growing senior care organizations, with locations across Milwaukee, Portage, and beyond. Through Saber Group Homes, we offer intimate, family-style group home care. Through UnityCare Assisted Living, we provide licensed, professional assisted living for those who need a higher level of support.

Across both brands, our commitment never changes. We hire people who care deeply, we build environments that feel warm and safe, and we treat every resident like a member of our own family. We are family-owned, Wisconsin-rooted, and growing, but no matter how big we get, we will never lose sight of why we started.

We accept MCO and Family Care, offer memory care and dementia support, and surround residents with daily activities, nursing care, and a team that genuinely loves what they do.

Saber Group Homes was the foundation of everything we built. Each location is intentionally small, so every caregiver knows every resident by name, by preference, and by story. We offer 24/7 qualified staffing, home-cooked meals, medication management, memory care, in-house doctors and physical therapy, and a level of personal attention that simply isn’t possible in larger facilities.

Sara Izgelov

Founder and Chief Executive Officer

Sara serves as Chief Executive Officer and provides strategic leadership for the organization. She oversees long-term planning, operational excellence, staffing initiatives, quality assurance, and organizational growth. Through her leadership, the company remains focused on delivering compassionate, resident-centered care while continuing to expand its services and communities throughout Wisconsin.

Rob Knoll

Founder and Chief Operating Officer

Rob oversees organizational operations, financial oversight, contract development, supplier and vendor partnerships, managed care organization (MCO) relationships, licensing coordination, budgeting, and strategic planning. He works closely with leadership teams, healthcare partners, and regulatory agencies to support operational excellence, organizational growth, and the continued delivery of high-quality care across all communities. Through his leadership and industry expertise, Rob plays a key role in strengthening partnerships, expanding opportunities, and ensuring the long-term success of the organization.
Yehuda Abramov

Yehuda Abramov

Owner & Chief Admissions Officer

Yehuda leads admissions, referral development, healthcare partnerships, and community outreach initiatives across the organization. He works closely with hospitals, rehabilitation centers, social workers, case managers, and families to ensure individuals receive the care and support they need. As one of the primary public-facing leaders of the company, Yehuda focuses on building strong relationships throughout Wisconsin’s healthcare community while helping drive organizational growth, occupancy, and resident satisfaction. His commitment to accessibility, responsiveness, and relationship-building has helped establish lasting partnerships and support the continued expansion of the organization.
David Abramov

David Abramov

Owner & Director of Acquisitions and Strategic Growth

David leads the organization’s acquisitions, expansion efforts, and long-term growth initiatives. He is responsible for identifying new opportunities, evaluating investments, conducting market research, and supporting strategic business development. Through careful analysis and planning, David helps position the organization for continued growth while expanding access to quality senior living services throughout Wisconsin and beyond.

Owner & Director of Administrative Services

Akiva oversees administrative operations, internal systems, organizational processes, documentation management, and backend support functions. His attention to detail and commitment to organizational excellence help ensure that communities operate efficiently while supporting staff, residents, and leadership teams across the organization.
Joseph Derxler

Joseph Derxler

Head Administrator | EAGLES REST

Joseph has spent more than 15 years in healthcare leadership, with a career spanning hospital operations, senior living, long-term care, and higher education, including teaching healthcare finance and operations at the University of Minnesota Duluth. At the heart of his leadership philosophy is the belief that exceptional healthcare begins with strong relationships, and throughout his career he has worked closely with clinicians, nurses, and administrative teams to build environments grounded in communication, trust, and collaboration. Outside of work, Joe brings that same value of connection into his personal life, spending time with his wife and daughters and finding peace through fly fishing, a hobby that reflects the same patience and focus he brings to his leadership.

Brittany Rauls

Regional director | PORTAGE

Brittany has dedicated over eight years to the healthcare field and currently serves as a Certified Administrator and Certified Nursing Assistant (CNA), combining hands-on caregiving experience with four years of administrative leadership and nearly two years with the Unity Care team. She is passionate about making a positive impact in the lives of others, providing compassionate, respectful, and person-centered care while fostering a supportive environment where residents and staff can thrive, guided by the belief that great leadership is built on integrity, teamwork, and open communication. Dedicated to continuous learning and professional growth, Brittany is proud to be part of a team committed to compassion, dignity, and excellence for every individual she serves.
Stephanie Quintero

Stephanie Quintero

Administrator | Greenfield AND BLAKEWOOD

Stephanie has dedicated her career to caring for older adults since earning her Certified Nursing Assistant (CNA) license in 2009, inspired by caring for her grandmother during her illness and discovering a lifelong passion for serving seniors with compassion and dignity. After more than a decade providing care in home health, skilled nursing, and hospitals, she earned her Associate Degree in Business Management and Administration, allowing her to combine hands-on experience with strong leadership, and as an Administrator she has successfully led communities through state surveys while maintaining high standards of quality and resident care. Stephani believes every resident deserves to feel safe, valued, and at home, leading by the simple philosophy that every resident should be treated as if they were your own loved one, and her commitment to compassionate care, staff development, and meaningful relationships makes her an essential leader within the Unity Care family.
Kellie Milczarski

Kellie Milczarski

Regional manager | GREENFIIELD AND BLAKEWOOD

Kellie began her healthcare career at just 20 years old as a housekeeper, where she quickly discovered her passion for caring for others, and over the past three years with Unity Care her dedication, leadership, and compassion have led to rapid advancement, from House Manager to Assistant Administrator and, just weeks later, Regional Manager. She is a Certified Nursing Assistant (CNA) and Certified Medical Assistant (CMA) with an Associate Degree, bringing both clinical knowledge and exceptional leadership to her role overseeing daily operations across Vila of Greenfield and Blakewood House, where she leads activities and resident engagement, ensures the highest standards of safety and quality of care, and plays a key role in hiring, mentoring, and developing staff. Known for her positive energy and ability to inspire those around her, Kellie believes great care starts with a compassionate team, and she is passionate about teaching caregivers to treat every resident like family, creating an environment where residents feel loved, respected, and truly at home.
